Not always. The length and diameter must fit your frame and brake type, for example a U-Brake setup. Pre lubricated housing can make the brake feel smoother.
Keep them clean and dry. If the cable feels stiff, add a small amount of PTFE lubricant unless your housing came pre lubricated.
Yes. Cracked or crushed housing makes the brake weaker and can cause the cable to fail, so replace it as soon as you notice damage.
